The practice is quite simple: Put a required "equity pause" in the agenda of your meetings. This means, at a certain time (e.g. 1/2 way through the meeting) you consciously pause the conversation and ask a question like "Are we considering all views & perspectives right now (e.g. women, minorities, etc.) in this meeting/solution/etc.?"
